1) Please download and complete the attached membership application HERE.  (This is a Microsoft Word Form.  Email Mike Chrisman if you need a different format.)

2) Once completed, save the document to your hard drive and forward it to mchrisman513@gmail.com   OR, print and mail it with your check (payable to BLSA) if paying by check.

3) Select Method of Payment:

A)Pay Online Using your Credit or Debit Card and Pay Pal.
Select membership level from the drop down box below:

NOTE:  You do not need to have a Pay Pal account or register with Pay Pal to use this feature.
While checking out, select the orange "Check out with Pay Pal" button, then"Don't have a Pay Pal account?" on the next screen.

The 'Add to Cart' button below takes you to the Pay Pal website to complete the payment process.
